IF YOU ARE AT THE PHARMACY NOW and are told the following:

  • Card is expired (our cards don’t expire)
  • Card doesn’t work

Have the Pharmacist call the Pharmacy Help Desk at 800-329-0988. (This is a Pharmacist ONLY number)

FOR EVERYTHING ELSE:

Please use this form to notify us if things did not go as expected at the pharmacy. 

If an error was made in pricing, you will be able to be reimbursed at the pharmacy. Don’t delay…the time frame is dictated by the pharmacy, and is typically only 1 to 2 days. We will need you to furnish information in a timely basis. If you have completed the form, please do not leave a voicemail message unless you have not heard a response from us within 48 hours or 2 business days.

If troubleshooting is due to pricing, WE WILL NEED THE FOLLOWING INFORMATION FROM YOU WHEN WE CALL YOU:

  • Member ID from card
  • Plan Group number from card
  • Prescription # from medication bottle
  • Date of birth 
  • Medication name 
  • Medication strength
  • Pharmacy name
  • Pharmacy location
  • Pharmacy phone number
